Can read from, but cant write too mySQL dbase

Hi all, hope this is the right place to post this....

ive uploaded a php page that writes an entry to a mySQL dbase as long as the entry does not already exist in the dbase. the read part works ok (checks if the entry is already in the dbase) , the write part does not.

it works fine on my localhost using wamp. any ideas what could be causing this?
